The clean-based ulcer is seen most often, in 42% of cases, and rebleeding risk is 5% or less.12 Because of the low rebleeding risk, the clean-based ulcer typically is not treated endoscopically. Instead, the patient should be treated medically. The clinician should test for Helicobacter pylori H pylori ; infection and treat the infection if the test is positive. However, patients diagnosed with an adherent clot at endoscopy have variable rates of rebleeding. The role of endoscopic treatment in these patients has been controversial. Recent randomized trials designed to evaluate endoscopic therapy for adherent clots have shown that combining endoscopic therapy with PPIs is superior to medication alone for prevention of recurrent bleeding. Future studies are anticipated.13, 14 The nonbleeding visible vessel and the actively bleeding lesion cause the greatest concern among gastroenterologists, because the risk of rebleeding is quite high for each. The nonbleeding visible vessel is seen in 17% of cases and has a rebleeding risk of 43%; the actively bleeding lesion has a prevalence of 18% and a rebleeding rate of 55% to 90%. These 2 lesions typically are treated with traditional endoscopic therapies: injection and heater probe or bipolar electrocoagulation. In the most common treatment, the physician injects the base of the ulcer with epinephrine, which serves as a vasoconstrictive agent and offers a tamponade effect. Negative Acts among Business Professionals: How the Business Context May Contribute to Indirect Forms of Aggression Denise Salin Department of Management and Organisation Swedish School of Economics and Business, Helsinki, Finland Negative acts and both direct and indirect forms of aggression in the workplace have lately received increasing attention. However, a majority of the studies have been conducted in the health care sector or in routine jobs in administration. In contrast, employees in highly careeroriented, knowledge intensive jobs have been highly neglected. The aims of this study were to describe the prevalence and forms of negative acts in a business context, more precisely, among business professionals and to analyse how the particular characteristics of the business context may contribute to these negative acts. A survey among members of a professional organisation for business professionals revealed that almost one-fourth of the respondents had been subjected to at least one of 32 predefined negative acts during the past 12 months from their colleagues, superiors or subordinates. The most common forms of negative acts were indirect forms of aggression and typically clearly work-related, for example, ignoring somebody's views and opinions, withholding information in order to make their work more difficult or giving somebody impossible tasks, thus setting them up to fail. The results of both the survey and stories written by business professionals further indicated that internal competition, and in particular a highly politicised climate, seemed to contribute to negative behaviour among business professionals. In fact, negative acts often seemed to be a way of boosting one's own career or "protecting" oneself against rivals. Structural factors in the business context also seemed to affect the perception of negative acts as women and employees in lower position felt more badly affected by such negative acts.

The use of neoadjuvant androgen deprivation therapy ADT ; prior to the treatment of localised prostate cancer either by radical prostatectomy or radical radiotherapy has been established in clinical practice for almost 20 years. There is a published European consensus statement that states, "neoadjuvant treatment for three months before radical prostatectomy does not improve outcome in terms of biochemical evidence of disease" For neoadjuvant ADT prior to radiotherapy, there are at least two large RCTs with long term follow up which indicate about a 10% absolute difference in survival rates after 5 years follow up. In the European Consensus document it is stated that the combination of external beam radiotherapy and hormonal therapy has a proven beneficial impact on survival in selected groups of patients those with significant risk of nodal involvement who receive treatment to the prostate and pelvis ; it is recommended that ADT should be started at least one month before radiotherapy but that the optimal duration of therapy remained unknownwith total treatment periods neoadjuvant plus adjuvant ; of between 6 and 36 months in use in different European countries. In practice duration of adjuvant use seems to be dictated by patient risk status in this country.

He believes it works by mobilising inorganic phosphate so it passes out in the urine and indeed urine samples of patients started on Guaifenesin show large increases in the excretion of phosphate. This mobilisation of phosphate may initially cause worsening of symptoms and so he recommends starting on low doses and building up gradually. Analysis of 264 patients showed improvement at different levels of treatment. Twenty percent of patients improved on 300mg twice daily. Fifty percent improved on 600mg twice daily. Twenty percent improved at 900mg twice daily. Ten percent required 2400mg Guaifenesin or more. Guaifenesin does appear to be extremely safe, has no listed side effects and is widely used as a cough linctus for children. Rarely patients get slight nausea, hyperacidity or sometimes rash. The cost of the Guaifenesin will be 11.00 per 100 400mg tabs ; . Initially treatment sometimes makes symptoms worse, as does increasing the dose. Amand reckons that regression with Guaifenesin occurs at the rate of about one year for every two months of treatment.
