SSRIs are metabolized by the cytochrome P450 system in the liver and may inhibit individual isozymes within the cytochrome P450 system. Isozymes 2D6 and 3A4 are the most common isozymes involved with interacting drugs. Fluoxetine and fluvoxamine are the most potent inhibitors of CYP3A4 5 and fluoxetine and Paxil are the most potent inhibitors of CYP2D6, making interactions more likely. SSRIs are significantly protein bound. Administration of other medications that are protein bound may cause displacement of either the interacting drug or the SSRI increasing free concentration. Warfarin and digoxin have documented interactions in this category. Serotonin syndrome is a complication of therapy with serotonin elevating drugs. Administration of other medications elevating serotonin will potentiate this condition. MAOIs, tryptophan, amphetamines, other stimulants, and other antidepressants have been identified as potential interacting drugs. Administration of fluvoxamine with thioridazine, terfenadine, astemizole, cisapride, or pimozide is contraindicated. In some cases where serious ventricular arrhythmias, qt prolongation, and torsades de pointes have occurred when cisapride was taken in conjunction with one of the cytochrome p450 3a4 inhibitors, elevated blood cisapride levels were noted at the time of the qt prolongation.

Cisapride is not currently licensed in the UK for use in children under 12 years of age. A and the recent CSM MCA bulletin1 manufacturer's Summary of Product Characteristics now specifically contraindicates the use of cisapride in neonates born before 36 weeks gestation for up to 3 months after birth because of an association with extended QT interval and because drug handling in this group may be immature. The bulletin states that there are insufficient data to support the use of cisapride in older children less than 12 years of age. It had been hoped that this issue would have included a report on two major meetings taking place in London and Edinburgh on this topic. However at the time of writing the Edinburgh meeting has still to take place. A report on both meetings will hopefully be in the next issue. In the meantime the policy for cisapride usage at Alder Hey is reproduced below. Cisapride has been used at Alder Hey for several years in babies, infants and children of all ages, although the use in preterm infants has been small. Cisapride is licensed for use in children from birth upwards ; in many other countries but many now caution about use in preterm infants. The CSM MCA have not made available the data presented by the manufacturer in support of a UK product licence application but the RCPCH Medicines Committee hopes to obtain this data and to pass comment. Action A small group of Alder Hey clinicians2 was convened to advise the Drug and Therapeutics Committee on the use of cisapride for Alder Hey patients. They reviewed the literature available and reflected on current practice. Recommendations 1. the decision to use cisapride should be taken by a consultant after consideration of the risks and benefits which should be discussed with the parents. The circumstances should be documented in the case notes. 2. cisapride should only be used for patients with proven gastro-oesophageal reflux on one or more of the following investigations upper GI contrast study hour oesophageal pH 24 monitoring endoscopy milk scan which has not responded to conventional measures such as feed thickening, antacids.
